Intramuscular (in terms of injections) is to inject into muscle. The only standard case in which intramuscular injections might be used in a body modification would be to administer local anesthesia during a tongue splitting procedure, although many people say the injection is more painful than the splitting.

In terms of body piercing, intramuscular piercings are piercings which pass through muscle. At the "simple" end of the scale, this includes tongue piercing, cheek piercing, labrets, and so on, and at the more "intense" end of the scale, includes bicep piercing, forms of deep chest piercings, Sprung Piercings and others.
