A Scalpelled Labret is an initial large gauge labret cutting.

Labrets can be started off at a large gauge by cutting across the lip using a scalpel. This cutting is almost always "with the grain" of the muscle (horizontally) but can be done vertically as well. 3/8" to 1/2" is the normal starter size, but plugs as large as 2" and bigger have been inserted initially.
